Quick answer: fibrosis after lipo or a BBL is firm, lumpy scar tissue that forms under the skin as the treated area heals. It tends to show up a few weeks after surgery, often around weeks 2 to 6, and feels hard and fixed in place rather than soft and puffy like normal swelling. Gentle lymphatic drainage, started early and done consistently with your surgeon's OK, can help control the swelling that feeds fibrosis and ease tightness. It won't dissolve established scar tissue overnight, and any new, hard, painful or hot lump should be checked by your surgeon first.
"Is this hard spot normal?" is one of the most common questions in the weeks after lipo. Here's how to tell what you're feeling, what helps, and when it's not a massage problem at all.
Fibrosis is excess scar tissue. When fat is removed or moved, your body lays down collagen to heal the tunnels left behind. If swelling lingers and fluid sits in the tissue, that healing can get thick and uneven, leaving hardened areas, ridges or lumps under the skin.
It's a normal part of healing to some degree. The goal of aftercare is to keep it from becoming more than that.
Usually a few weeks in, not the first days. A 2018 Brazilian clinical trial on abdominal liposuction and tummy tuck patients saw fibrosis appear around days 16 to 19 after surgery. Its authors note collagen production peaks between days 6 and 17, and that lingering swelling favors fibrosis.
A board-certified plastic surgeon in Houston describes a similar arc: swelling and firmness are common in weeks 1 to 3, lumps often start softening between weeks 4 and 8, and final contours take 6 to 9 months.
Feel the area and watch it over a few days.
| Normal swelling | Fibrosis | Seroma | |
|---|---|---|---|
| Feels | Soft, spongy, spread out | Firm, rope-like or lumpy | Squishy, like a water balloon |
| Changes | Up and down through the day | Stays the same in one spot | Can grow, may feel fluid move |
| Skin surface | Smooth but puffy | Uneven, dimpled or ridged | Raised bulge |
| Typical timing | Peaks early, eases over weeks | Often weeks 2 to 6 and later | Often days 7 to 10, after drains |
| Who handles it | Compression, movement, lymphatic drainage | Your surgeon's plan, often with lymphatic drainage | Your surgeon. It may need draining with a needle. |
A seroma is a pocket of clear fluid. Many go away on their own, but some need a clinician to drain them. Never try to drain a seroma at home, and a massage therapist should never drain fluid from an incision. In Florida, that's outside a massage therapist's scope.
A hematoma is a collection of blood under the skin. It's usually dark, tender and appears early. That's also a call to your surgeon.
Call your surgeon right away if you notice:
Call 911 for sudden shortness of breath, chest pain that's worse when you breathe in, a racing pulse, fainting or coughing up blood. Mayo Clinic lists these as possible signs of a pulmonary embolism.
If you're unsure, ask your surgeon first and book the massage after. We'd always rather reschedule than work on something that needs a doctor.
It helps most with prevention, and it can make established fibrosis more comfortable. Here's what the research actually shows:
Our honest take: the evidence is small, and much of it comes from Brazil, where post-op drainage is routine. It supports gentle, consistent sessions for swelling and comfort. Nothing shows that massage breaks up mature scar tissue. Be careful with anyone who promises that.
At Loly Spa, it's slow and gentle. Your therapist will:
Sessions are $120 each, and we can come to your home, hotel or recovery house anywhere in greater Miami. That matters after a BBL, when sitting in a car is a problem. See our post-op lymphatic drainage service.
Mild firmness often softens over a few months as healing finishes, especially with compression and movement. Established, hard fibrosis can last much longer. Surgeons may treat stubborn cases with procedures like ultrasound-guided revision months after the original surgery.
Swelling takes time. ASPS says it may take several months for liposuction swelling to fully settle, so don't judge your results at week four.
Not usually. Most firmness softens over months. Some hard areas can persist and may need your surgeon's help.
It can feel tight, tender or numb. Sharp, worsening pain, heat or fever isn't typical fibrosis. Call your surgeon.
There's no fixed number. The study above used 12 sessions, and many surgeons ask for several a week early on. See how many post-op sessions you need.
Only if your surgeon shows you how. Pressing hard on healing tissue can make things worse.
It can, mostly in the liposuction areas like the abdomen, flanks and back. The grafted area itself shouldn't be massaged unless your surgeon says otherwise.
